what the checker should catch that it currently doesn't, read off the failures the agent shipped past it

the check phase is the cheapest place to add a guardrail and the easiest place to over-engineer one. i'm collecting the small list of things that landed in production despite the check, and i'd love to compare lists with anyone else running an automated review pass.
